About us and our History
Since we first opened our doors in September 2012, Centralia Physical Therapy has grown into a trusted provider of rehabilitation services. Over the years, we’ve expanded our reach to better serve our communities. In September 2017, we opened our Chehalis facility, a 6,200-square-foot clinic equipped with state-of-the-art technology, private treatment rooms, and flexible scheduling options, including early morning and evening hours. We also make it easy for new patients to get started with same-day or next-day appointments and accept contracts with major insurance carriers.
In 2024, CPT completed construction on two additional facilities, welcoming patients to our new clinics in Grand Mound and Castle Rock. With four active clinics now serving the region, we are excited to continue expanding access to our high-quality care. Currently, we are developing our fifth location in Tumwater Valley, set to open in February 2025, where we’ll bring the same level of expertise and personalized attention that our patients have come to expect.
Across all CPT locations, we provide care for a wide range of conditions, including pre- and post-operative orthopedic issues, extremity dysfunction and pain (shoulder, elbow, hip, knee), neck, back, and sacroiliac pain, sports injuries, and workers’ compensation cases (L&I and work conditioning). We also specialize in geriatrics, foot and ankle injuries, tendinopathies, mobility evaluations, hand and wrist rehabilitation, custom hand splinting, vestibular conditions, motor vehicle accidents, and running analysis.
Our growth reflects our unwavering dedication to providing individualized care and innovative treatment techniques to help our patients achieve optimal health and well-being.
